Dear Parents,

This letter is to inform you of the Hooksett School District’s policy for students entering sixth grade in the 2021-2022 school year, HSD File: JLCA.

THE HOOKSETT SCHOOL DISTRICT REQUIRES A COMPLETE MEDICAL EXAMINATION (PHYSICAL) BEFORE ENTRANCE INTO SIXTH GRADE. (7th and 8th Grade Parents See below)

A physical done after August 31, 2020 will be considered current.  If for insurance reasons a physical cannot be done by August 31, 2021 a copy of the appointment card of the impending physical must be given to the school nurse prior to the start of the new school year.

At the time of the exam all immunization records must be reviewed and brought up to date as required by New Hampshire law.   

If your child is entering 7th or 8th grade you can also send in an updated physical exam and immunization record. Children entering 7th grade should have a Tdap booster vaccine before returning to the school this fall.

The medical examination and updated immunization record are also necessary for participation in interscholastic sports. Students will not be allowed to try out or participate in interscholastic sports if a copy of the updated immunization record, physical or appointment card is not on file.  A parent note is not acceptable.

Please make a copy of the physical form and updated immunization record (for your records) prior to submitting it to the school.

The completed physical exam form (done 8-31-20 or after) with the updated immunization record must be returned to the Cawley School Nurse prior to the first day of school of the 2021-2022 school year.

Failure to comply with the above policy may lead to exclusion from school.
